  • Partner Express Verification Form: Non-DES partners should use this form to provide information about companies that must be verified in AJC. The DES team will check this information before performing verification. The person completing this form confirms all information submitted is valid.

Information on Unemployment Insurance (UI) benefits is constantly evolving. To find the most up-to-date information on Arizona Unemployment benefit programs, visit the Unemployment Insurance Benefits page. If clients have questions about the UI programs, please direct them to call the UI call center at (877) 766-8477 or visit the Unemployment Insurance Benefits page. UI staff are not stationed in DES or local area offices and can only be reached by calling the call center.
